Inspector Notes

An unannounced monitoring inspection was conducted today from 10:10am-1:00pm. There were 12 children directly supervised by 4 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, and policies were inspected. Children were observed completing a starfish art project and playing freely. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 4 records of immunizations were not signed by a physician.

Evidence:
1. The immunizations obtained prior to start date for child #1 did not contain a physician's signature.
Plan of Correction: Immunizations with a physician's signature were obtained on 8/31/2023
Standard 8VAC20-780-550-P
Based on record review, 2 of 4 written records of injuries did not include all required information.

Evidence:
1. An injury report dated 1/9/2024 did not contain the time when parents were notified. The injury report did not contain two signatures from either two staff members or a staff member and one parent. The record contained one staff signature.
2. An injury report dated 2/14/2024 did not contain the time of injury.
Plan of Correction: Injury report requirements will be reviewed with staff.
