Inspector Notes

Today an unannounced Renewal Inspection took place between the hours of 12:35pm and 2:00pm. There were 3 classrooms observed with a total of 56 children with 8 staff within the supervision guidelines. The children were observed playing on the playground and taking a nap. A complete inspection of the physical plant, children and staff records, board member records, evacuation drill log, 2 emergency medications, and emergency materials were observed during this inspection. Based on a review of staff records, 1 out of 3 staff did not have a tuberculosis screening at least every two years from the date of the first initial screening or testing.

Evidence:
1. Staff #3's (doh: 9/2016) most recent documentation of tuberculosis screening on file was dated 9/29/2020.
Plan of Correction: The staff member has documentation of getting the tuberculosis screening on 10/11/2022, however is still waiting on the results. Once received, we will place the documentation in the staff member's file.
