Inspector Notes

An unannounced renewal inspection was conducted from approximately 10:25 am-12:10 pm. At the time of entrance, 42 children were in direct care of 6 staff members. Children were observed engaged in small motor activities, manipulatives, engaged with staff and transitioning to outdoor play time. The site was clean, organized and contained a varied supply of developmentally appropriate materials.

A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, required posted documents, and medications were reviewed. Rooms were measured and restrooms were reviewed for compliance.

Based on record review and interview, it was determined that the center did not ensure that board officers that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Central Registry search request and the result of an out-of-state Sex Offender Registry search request.
Evidence:
1) The record for Staff #3 did not contain documentation of an out-of-state Central Registry search request and the result of an out-of-state Sex Offender Registry search request. Staff #3 had declared in their sworn statement having lived out of the state of VA within the last 5 years.
Plan of Correction: In the process of getting out-of-state information.
Standard 22.1-289.036-B-2
Based on record review, the center did not ensure to obtain fingerprint-based criminal history check determination letter prior to first day of employment for one staff person.
Evidence:
1) The record for Staff #2 (start date 08/16/2021) had a fingerprint-based criminal history check determination letter dated 08/26/2021.
Plan of Correction: Staff member was employed but class for children did not begin until Aug 25, 2021. Staff member began working with children Aug 30th. Will make sure new staff doesn't start before we have all the paperwork.

Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain the work phone number of one custodial parent.
Plan of Correction: Talked to parent. Parent working from home - using cell number listed. Corrected form.
