Inspector Notes
A monitoring inspection was conducted on 11/28/2022 between the hours of approximately 1:30 pm and 2:45 pm. There were 42 children present, ranging in ages from 3 to 5 years, with 5 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
7Evidence:
1) The record for Child #2 (start date 09/07/2021) did not contain the results of a physical examination.
Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. The Tb screening form in the record for Staff #1 (start date 08/2022) was dated 03/25/2022.
Evidence:
1) On the date of inspection, the records for Staff #1 and Staff 2 (start date 08/2022) did not contain documentation of having completed the Virginia Department of Education-sponsored orientation course.
(CPR), as appropriate to the age of the children in care, within 30 days of employment.
Evidence:
1) On the date of inspection, the record for Staff #2 (start date 08/2022) did not contain documentation of having completed orientation training in first aid and cardiopulmonary resuscitation.
Evidence:
1) On the date of inspection, the most recent inspection report available was dated 06/10/2021.
Evidence:
1) The record for Child #1 did not contain the work phone number and place of employment of each parent who has custody.
2) The records for Child #2 and Child #3 did not contain the work phone number of each parent who has custody.
Evidence:
1) The daily attendance record did not document the departure of each child in care as it occurs.
