Inspector Notes

A monitoring inspection was conducted on 11/28/2022 between the hours of approximately 1:30 pm and 2:45 pm. There were 42 children present, ranging in ages from 3 to 5 years, with 5 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-140-A
Based on record review, the center did not ensure that each child have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
1) The record for Child #2 (start date 09/07/2021) did not contain the results of a physical examination.
Plan of Correction: Parents have been asked to provide a physical.
Standard 8VAC20-780-160-A
Based on record review, documentation of a negative tuberculosis (Tb) screening for one staff person did not meet all requirements.
Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. The Tb screening form in the record for Staff #1 (start date 08/2022) was dated 03/25/2022.
Plan of Correction: staff member has been asked to provide this.
Standard 8VAC20-780-240-A
Based on record review, staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1) On the date of inspection, the records for Staff #1 and Staff 2 (start date 08/2022) did not contain documentation of having completed the Virginia Department of Education-sponsored orientation course.
Plan of Correction: Staff will do the orientation course.

Based on record review, the center did not ensure that staff complete orientation training in first aid and cardiopulmonary resuscitation
(CPR), as appropriate to the age of the children in care, within 30 days of employment.
Evidence:
1) On the date of inspection, the record for Staff #2 (start date 08/2022) did not contain documentation of having completed orientation training in first aid and cardiopulmonary resuscitation.

Based on documentation review, evidence of an annual health department inspection report was not available to review.

Evidence:
1) The record for Child #1 did not contain the work phone number and place of employment of each parent who has custody.

Based on documentation review, the written record of daily attendance did not meet all requirements.
