Inspector Notes

A renewal inspection was conducted on 12/12/2023 between the hours of approximately 10:30 am and 11:45 am. There were 46 children present, ranging in ages from 3 years to 6 years old, with 5 staff supervising. Children were observed while engaged with puzzles, tracing, eating snack, putting materials away, and coloring.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-280-B
Based on observation, hazardous substances were not kept in a locked place that prevents access by children.
Evidence:
1) On the date of inspection, a bottle of hand sanitizer was observed on top of a bookshelf outside of room 207. This bottle was accessible to children.
Plan of Correction: The bottle was removed.
Standard 8VAC20-780-40-M
Based on documentation review and interview with staff, the center did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan.
Evidence:
1) On the date of inspection, the allergy list available for review in room 207 was dated for the school year 2022-23. Staff in the room stated that the information in the list is not current.
Plan of Correction: New current list was handed out and posted.
Standard 8VAC20-780-510-N
Based on medication review and interview with staff, the center did not keep a record of medication given children.
Evidence:
1) On the date of inspection, Staff #4 stated that medication for Child #1 has been administered but a record documenting the medication administration has not been kept.
Plan of Correction: Teachers will keep a log of when the medication is given.
Standard 8VAC20-780-60-A
Based on record review, children's records did not contain all required information.
Evidence:
1) The records for Child #2 and Child #3 did not contain the work phone number of one or two of the parents who have custody.
2) The record for Child #4 did not contain the work phone number and place of employment of each parent who has custody.
Plan of Correction: Will follow up with parents to make sure that forms are completely filled out.
