Inspector Notes
A renewal inspection was conducted on 12/12/2023 between the hours of approximately 10:30 am and 11:45 am. There were 46 children present, ranging in ages from 3 years to 6 years old, with 5 staff supervising. Children were observed while engaged with puzzles, tracing, eating snack, putting materials away, and coloring.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
4Evidence:
1) On the date of inspection, a bottle of hand sanitizer was observed on top of a bookshelf outside of room 207. This bottle was accessible to children.
Evidence:
1) On the date of inspection, the allergy list available for review in room 207 was dated for the school year 2022-23. Staff in the room stated that the information in the list is not current.
Evidence:
1) On the date of inspection, Staff #4 stated that medication for Child #1 has been administered but a record documenting the medication administration has not been kept.
Evidence:
1) The records for Child #2 and Child #3 did not contain the work phone number of one or two of the parents who have custody.
2) The record for Child #4 did not contain the work phone number and place of employment of each parent who has custody.
