Inspector Notes

An unannounced renewal inspection was conducted on 4/12/2022. There were 27 children directly supervised by 6 staff and parent volunteers. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing with manipulatives, engaged in circle time, sensory play, and counting activities. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 2 of 5 children's record did not contain the place of employment for each parent.
Evidence:
The records for Children #4 and #5 do not contain the name or work number for either parent's employer, or a documentation that the parentis not employed.
Plan of Correction: One parent was not employed upon enrollment, we will have that parent complete the current information. The other parents do not work and we will have them note it on their child's paperwork.
