Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated and conclude on September 27, 2021. The director was contacted by telephone to initiate the inspection. There were 12 children present and three staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed three staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, in one of three staff records reviewed, the center did not ensure that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.

Evidence: The director confirmed that the staff health report for Staff #3 was more than one year old and was dated 9/8/2020.
Plan of Correction: Staff has been given the form and will have it completed by the doctor.
