Inspector Notes

An unannounced renewal inspection was conducted on 06/03/2024 from 11:15 a.m. to 2:45 p.m. There were 86 children present, ranging in ages from six month to five years old with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of nine child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

REPEAT VIOALTION
Based on review of records, the center failed to receive, before the first day of employment, Out-of-State Criminal History background check for staff who had lived out of state within the past five years.
Evidence:
1. Staff #4 indicated on the sworn disclosure that hey had lived in Rhode Island withing in the last five years. Staff #4?s start date was 05/20/2024. There was no Out-of-State Criminal History check for Rhode Island on file for Staff #4.
Plan of Correction: Staff #4- the FBI Criminal Background Check, which included a review or the Sex Offender Registry, was completed prior to start date. The out-of-state Criminal History Check for Rhode Island was received (via mail) on 6-4-24, The employee was never left alone while supervising children.

Based on observation, the center failed to ensure that over-the-counter skin product are not kept or used past their expiration date.
Evidence:
1. In the older 2?s classroom, there was one container of Boudreaux?s Butt Paste that expired on 05/2024.
2. In the Prep B classroom, there was one container of Babygonic?s sunscreen that had expired on 12/2023 and one container of Banana Boat sunscreen that had expired on 10/2023.
Plan of Correction: All expired items were removed and sent home with the parents on day of visit. Teachers will check all skin products received by parents to ensure all items are not expired
