During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

Unannounced inspections were initiated on October 9, 2025, November 3, 2025 and completed on December 3, 2025 with a local agency in response to a complaint received by the licensing office on October 1, 2025 relating to allegations of administration, supervision, special care provisions, injury reporting and records. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, interviews, tour of the facility, and observations. Additional interviews were conducted off-site.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, a violation that was not reported was found related to staffing and supervision standard and was documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 12, 2025. A POC submitted after this date will not appear on the public website.

In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

For at least three months, there was no documented lead teacher for the Toddler classroom.
Plan of Correction: The staff member who was overseeing the classroom at the time of the licensing inspection is no longer employed by the Center. The Center is currently working in collaboration with the Education Department and management team to ensure that current staff assigned to Lead Teacher roles meet all required training and qualification standards. The identified staff member has completed 20 hours of professional development within the past 12 months and is enrolled in additional required training to support instructional quality and promote a safe, nurturing, and developmentally appropriate learning environment.

The Center will continue to monitor staff qualifications and training records to ensure compliance with Lead Teacher requirements prior to classroom assignment. Administrative staff will conduct regular reviews of personnel files and provide ongoing oversight to ensure required training is completed and documented in accordance with licensing regulations.
