Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on February 23, 2024. There were 26 children ages 18 months to5 years old present under the direct care of 8 staff. The Licensing Inspector reviewed 5 children?s records and 11 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Center failed to ensure staff who will be alone with, in control of, or supervising children undergo a background check every five years.

EVIDENCE:
1. Staff #6 did not have an updated Background Check (Fingerprint) completed every 5 years. Previous Fingerprint Background Check was completed 5/18/2018.

2. Staff #7 did not have an updated Background Check (Fingerprint) completed every 5 years. Previous Fingerprint Background Check was completed 5/23/2018.

3. Staff #8 did not have an updated Background Check (Fingerprint) completed every 5 years. Previous Fingerprint Background Check was completed 6/28/2018.

4. Staff #11 did not have an updated Background Check (Fingerprint) completed every 5 years. Previous Fingerprint Background Check was completed 8/29/2018.
Plan of Correction: Center Director will schedule appointments for each staff person to complete an updated Fingerprint background check. Documentation of the findings will be maintained in the staff record. Director will submit documentation to the Licensing Inspector to confirm background checks have been completed.

Based on inspection, the Center failed to have a carbon monoxide detector installed in each building that was built before 2015 and that houses a child day program that serves preschool-age children.

EVIDENCE:
A carbon monoxide detector was unavailable.
Plan of Correction: Center will purchase and install a carbon monoxide detector.
