During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site code compliance inspection was initiated on (2-4-2026) and completed on 2-9-2026. The on-site inspection began at 10:05am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 41 children present with 6 staff. The inspector reviewed 5 children?s records and 9 staff records on-site. This inspection included
document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable code sections, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2-18-2026. A POC submitted after this date will not appear on the public website

The center did not ensure, to request results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years prior to the employment.
1.Staff #1, who has been working for more than 5 months, has lived in two different states outside of Virginia within the last 5 years. Staff #1 did not have the results of a criminal check, sex offender check and a child abuse and neglect search for one state and did not have the sex offender check and child abuse and neglect search for another state .
2. Staff #7, who has been working for more than 5 months, did not have the results of a sex offender check and a child abuse and neglect search for one state where they had lived in the last five years.
3. Staff #9, who has been working for more than 5 months, did not have the results of a sex offender check and a child abuse and neglect search for one state where they had lived in the last five years.

The center shall require from the person enrolling the child information regarding previous child day care. The records for child #1, 2, 3, 4, & 5 did not include previous child care and schools attended.
