Inspector Notes

A monitoring inspection was conducted on 8/2/2022. There were a total of 33 children with 10 staff members present. Children were observed playing baseball, uno, and building with legos. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on child record review, each child did not have a physical examination by or under the direction of a physician.
Evidence:
Child #1did not have a physical on file during the inspection.
Plan of Correction: We are going to tell the mom we need the information as soon as possible.

Based on child record review, each center did not maintain a record for each child enrolled containing two emergency contacts.
Evidence:
Child #4 was missing one of the two required emergency contacts.
