During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

AMENDED
An unannounced monitoring inspection was initiated on 10/22/2025 and completed on 11/7/2025 with a local agency in response to a self-report received by the licensing office on 10/15/2025, relating to supervision. The inspector reviewed 1 child?s records and 1 staff record on-site and electronically on 11/6/2025. This inspection included document review, a tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/14/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.
On October 15, 2025, Staff #1 was unaware that Child #1 (age 12 years old) had left the classroom and walked to the school?s playground. Child #1 was out of sight and sound supervision for approximately fifteen minutes.
Plan of Correction: Staff will ensure the care, protection and guidance of all children at all times. Center procedures will be reviewed to ensure all room entrances are able to be monitored effectively while maintaining other program operations. The child specific Plan of Action will be implemented consistently and will be revised to include additional strategies for monitoring the door, ensuring supervision of the child and providing additional care and support for the child to ensure their wellbeing. As a center team staff will participate in training on the Plan of Action, communication, safety and accountability procedures, and active supervision. Monthly center observations will be conducted by regional managers. Additional program wide trainings will be conducted in active supervision.

Children 10 years of age and older shall be within actual sight and sound supervision of staff.

Monthly center observations will be conducted by regional managers. Additional program wide trainings will be conducted in active supervision.
