Plan of Correction: A thorough review of all onboarding procedures will be conducted to ensure all steps are completed in compliance with timeframe requirements. Updated onboarding procedures and responsibilities will be communicated to all hiring managers. Additional steps will be put in place to identify incoming employees who have resided out of state in a timely manner and to ensure incoming employees have technical assistance in order to complete the reports accurately for submission and in compliance with the timeframe.

Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.
Staff #4 TB documentation did not indicate it was completed within 30 days of the hire date, as the date on the form was unreadable and blurry. Staff #4 had been employed for four months.
