During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 04/01/2025, as a part of the licensure period. The on-site inspection began at 12:20p.m. and ended at 1:30p.m. The inspector reviewed compliance in the areas listed above. There were 26 children present and3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/10/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-245-J-3
Any child for whom emergency medications have been prescribed shall always be in the care of a staff member who has medication administration training.

Child #1 who was present during the inspection, requires emergency medication. No staff with medication administration training was present.
Plan of Correction: Any child who is prescribed emergency medications will always be in the care of a staff who has MAT training. Additional staff will register to participate in MAT training. Center staff will work with their supervisor to ensure a MAT trained staff is assigned to the center as needed for center coverage.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were a total of 26 children present during the inspection. All 26 children were recorded that they had departed on the written record of attendance for 04/01/2025.
Plan of Correction: Center staff will ensure an accurate record of daily attendance is maintained at all times to ensure accountability for all children.
