Inspector Notes

An unannounced Renewal Inspection took place on 01/30/2023 between the hours of approximately 11:05 a.m. and 12:40 p.m. There was one classroom observed with a total of 12 children with two staff within the supervision guidelines. The children were observed cleaning up centers, participating in a phonics activity and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and staff interview, not every staff member submitted documentation of a negative tuberculosis screening.

Evidence: Staff #1 (start date: 07/01/2022) did not submit documentation of a negative tuberculosis screening.
Plan of Correction: I will get it done.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances were not kept in a locked place.

Evidence: There was one bottle of adhesive remover, one room spray, one bottle of dish soap (warning label says to keep out of reach of children), two containers of disinfectant wipes, one hand sanitizer, and one bottle of lotion (warning label says to keep out of reach of children) in an unlocked cabinet in the JK classroom.
Plan of Correction: Corrected during inspection.

Based on record review and staff interview, injury prevention procedures were not updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence: Staff #1 stated that the center had not reviewed documentation of injuries and updated injury prevention procedures annually.
Plan of Correction: We will start using the report and will schedule a meeting at the end of the school year to review any incidents and update.
