Inspector Notes
A monitoring inspection was conducted on 9/15/2022. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview. The program director provided plans of correction during the exit review.
Violations
3Evidence:
1. The wood mulch used as resilient surfacing under the large and small climbers, swings and the "mushroom" stepping stone balance was measured to be less than 6 inches in depth in many areas.
2. Staff 1 acknowledged that the depth of the wood mulch used as resilient surfacing under the above listed equipment is less than six inches deep in many areas.
Additional mulch will be ordered and spread under the cited equipment. Existing mulch will be raked back into place within the fall zones of the cited equipment.
Evidence:
1. There was one child in room 21 who was on a cot with a pillow. The pillow was not labeled with his name and did not have a removable cover.
2. There were two children on cots in room 14 with pillows that were not identified for individual use and did not have removable covers. Staff stated that she gave one of the two children a pillow from their quiet area.
All staff will be reminded that, if pillows are used for children over the age of two years, they must be labeled with the child's name and have a removable cover to be sent home weekly for laundering.
Evidence:
1. In the diapering area of the toddler classroom (room 17), there were diapers disposed in an open diaper can without a lid.
2. Staff 1 acknowledged that the diaper can did not have a lid and that diapers had been disposed of in this can.
All staff will be reminded that diaper disposal units are to remain covered at all times. The covers must be operated by foot or in such a way that neither the diaper or the hands of staff touch the disposal unit.
