Inspector Notes

A renewal inspection was conducted on 02/15/2023 . There were 120 children present, ranging in ages from 2 yrs to 4 yrs, with 35 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 child records and 7 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the Assistant Director during the exit interview.

Standard 8VAC20-770-60-C-2
Based upon review of staff records, the facility has not ensured that results of a child protective services central registry search are obtained at least every five years.
Evidence:
The most recent results of a central registry search for staff 7 were obtained 12/18/2017.
Plan of Correction: The facility responded with the following:
The CPS central registry search will be immediately requested for staff 7. The tracking form for background checks will be reviewed for accuracy to ensure updates are obtained in a timely fashion,
Standard 8VAC20-780-160-C
Based upon review of staff records, the facility has not ensured that results of a tuberculosis screening of staff are obtained at least every two years.
Evidence:
The most recent tuberculosis screening in the record provided for staff 8 is dated 02/10/2021.
Plan of Correction: The facility responded with the following:
Staff 8 was reminded of the need for a repeat tuberculosis screening. Her screening is scheduled for next week.
Standard 8VAC20-780-240-A
Based upon review of staff records, the facility has not ensured that the Va Department of Education sponsored orientation course is completed within 90 calendar days of employment.
Evidence:
The facility could not provide documentation that staff 3, employed since 8/29/2022, completed the required ten hour orientation course.
Plan of Correction: The facility responded with the following:
Staff 3 will be immediately required to complete the preservice orientation training. In the future, all staff will complete the training within 90 days of employ.
Standard 8VAC20-780-280-B
Based upon observation, the facility has not ensured that potentially hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
There was a container of Epsom Salts, labeled "warning" and "keep out of reach of children" in a curtained cupboard under the children's handwashing sink.
Plan of Correction: The facility responded with the following:
The Epsom Salts was immediately moved to a locked cabinet. Staff were reminded to check all items for hazard/warning labels and to keep all potentially hazardous substances in locked cabinets.
