Inspector Notes

A complaint inspection was conducted on .5/18/2023 and continued on 6/7/2023 in response to a complaint alleging insufficient staffing, inaccurate record keeping, staff/management interaction and facility cleanliness. The investigation included physical plant inspection, review of documentation and staff/management interviews. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and emergencies.

Information gathered during the inspection determined non-compliance with an applicable standard and a violation was documented on the violation notice issued to the program and discussed with Program Director during the exit interview.

Based upon review of records and interviews, the facility has not ensured that they maintain a written record of children's injuries in which entries are made the day of occurrence.
Evidence:
Interviews with staff and management revealed that the written record of an injury to child 1 on 3/31/2023 was not completed and signed by staff until 4/3/2023.
Plan of Correction: The facility responded with the following:
We have added Incident Report review and importance of writing up the report the day of the incident into our training for the summer staff and school year staff.
