Inspector Notes

A Subsidy Health and Safety inspection was conducted on on 9/2/21 and during the inspection there were 61 children ages five years old through ten years old in care with 8 staff. Children were observed participating in various activities in the classrooms, swimming and leaving on a field trip. Records were reviewed for four children and four staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements, were discussed during the exit interview, and are identified on the violation notice.

Violations

1
Standard 22VAC40-665-580-B
Based on a review of 4 staff records, it was determined that the vendor did not ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff, which shall include training on the following topics and training modules.

Evidence:
1. The record for staff #4 did not include any documentation that staff #4 had completed the Virginia Preservice Training for Child Care Staff.
2. Staff #5 confirmed that staff #4 had been employed more than 90 days and had not completed the Virginia Preservice Training for Child Care Staff.
Plan of Correction: The vendor responded: Staff #4 will complete the Virginia Preservice Training for Child Care Staff. Going forward all new staff will complete the Virginia Preservice Training for Child Care Staff within 90 days of employment.
