In each grouping of children there shall be at least one staff member who meets the qualifications of a program leader or program director. A teacher assistant (Staff B) was supervising a group of 17 school-age children by herself from around 2:45pm-5:15pm. Staff #B did not have documentation that she met the qualifications of the program leader position.
Plan of Correction: The director and program lead were both absent and the designated person in charge did not make the adjustment to place a lead with Staff A. Clear chain of command will be discussed with all staff to ensure each grouping of children have a program leader.

Behavioral guidance shall be constructive and intended to redirect children to appropriate behavior.
Staff did not redirect children to appropriate behavior when participating in the following behaviors:
1. Freely roaming between rooms in which they were not assigned to

4. One child flipped himself over in his chair

Plan of Correction: The director plans to provide classroom management training specific to school-age children and provide a more structured routine starting with when the children get off the bus and into the center.

The center's procedures for administering medication shall include methods to prevent use of outdated medication. A procedure that addresses methods to prevent the use of outdated medication was not included in the center's medication policy.
Plan of Correction: The director will discuss the requirements of adding methods to prevent outdated medication with the health and safety director. A policy and procedure will be developed to add to the center's policy on medication.

When centers choose to provide snacks they shall offer a variety of nutritious foods
and shall serve at least three sources of vitamin A and at least three sources of
vitamin C on various days each week. In addition a menu listing foods to be served for meals and snacks during the current one-week period shall be posted. The center did not have a menu of snacks posted. Staff stated they don't use a menu and that snacks are taken from the snack shelf, which only contained 1 type of snack.
