Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Seven groups of children were permitted by staff to exit the center's bus and enter the licensed center via a propped open door with no adult supervision.
1. Inside the door was a flight of stairs that led to the after school program.
2. Children had access to the large area hidden from the doorway under the stairwell and an unlocked door that led to unlit and unoccupied rooms.
3. When the groups of children arrived at the top of the stairs, the doorway to the afterschool program was also propped open with no staff to check them in.
4. The center is accessible to the public and located in a busy shopping area with a high volume of traffic.
Plan of Correction: Staff have been retrained on the policies regarding supervision and transfer of care
between the bus and the center. Effective immediately, no child is permitted
to exit a bus or enter the facility without direct staff supervision. All exterior doors
used during arrival are kept closed and monitored by assigned staff.
Children will be received directly from staff during the offload transition between the
bus and the center.
Standard 8VAC20-780-340-C
There shall always shall be on field trips when one or more children are present one staff who meets the qualifications of a program leader or director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The center sent 1 staff member, where 2 are required to local schools to wait for children until the bus arrives to take them to the center.
Plan of Correction: If the program is short on drivers, a proper
plan will be created to ensure the licensing standards are within compliance of the field trip standards.Two staff will be assigned to sit at the school to wait with the children, if needed. Pick-up does not occur unless staffing
requirements are met.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff. Seventy three children ages 5-9 years were not in sight and sound supervision of staff when they were dropped off in front of the center's building, when entering the building or while they walked up the staircase and into their classroom.
Plan of Correction: Staff have been retrained on the policies regarding supervision and between the transfer of care between bus and center. No child is permitted to exit a bus or enter
the facility without direct staff supervision.

Children 10 years of age and older shall be within actual sight and sound supervision of staff . Seventeen children, ages 10-12 years, were not in sight and sound supervision of staff when they were dropped off in front of the center's building, when entering the building or while they walked up the staircase and into their classroom.

Standard 8VAC20-780-340-I
Staff shall greet each child upon arrival at the center. Children in the afterschool program were not greeted by staff when they arrived at the center or when entering their classroom.
Plan of Correction: Designated staff will greet and check in each child upon arrival.
Children will not be able to leave the bus and enter the program until the staff have
verbalized that they are ready to receive the children.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's own policies and procedures are maintained in compliance with the required standards. The center's transportation policy states, "We will only release children in our care to an authorized adult." Ninety children on 7 different bus runs whom were transporting to the center's afterschool program were released off the buses and not received by an adult. The children entered the building via a propped open door with no adult supervision.

There shall be a communication plan between center staff and staff who are
transporting children or on a field trip. Seven groups of children were permitted by staff to exit the center's bus and enter the building via a propped open door. The bus staff did not communicate with the center staff to let them know children were entering the building, nor did they provide the center staff with the roster of children they transported and dropped off in front if the building.
Plan of Correction: Staff will ensure that when transporting children they communicate via radio with
the staff if they are not able to walk them inside, to ensure that the staff in the
program are ready to receive them properly and in accordance with our
policies and procedures
