Inspector Notes

An unannounced Code Compliance inspection was conducted with the Director on 1/24/24 from 10:00am to 11:00am. There were 47 children in care, ranging in age from two-years-old to five-years-old, supervised by 11 staff. Children were observed learning about the letter ?L? and painting a ?lollipop? craft project, participating in circle time, listening to a teacher read a book about a snowman, playing in the indoor playroom, and participating in a music class with drums. The required postings and six classrooms were observed. The playground could not be inspected due to rain and snow on the ground. Five child and five staff records were reviewed for required documentation. The program met the requirement for staff on-site with current CPR/First Aid training, Daily Health Observation training, and Medication Administration Training. REPEAT VIOLATION Based on review of five staff records, the program did not obtain the results of a national fingerprint-based criminal history record check for one staff prior to employment. Evidence: The fingerprint results on record for Staff #2, with an employment date of 10/10/23, were dated 12/1/23.
Plan of Correction: Fingerprint results were obtained and on file. Fingerprint results will be obtained prior employment.

REPEAT VIOLATION Based on review of five staff records, the program did not obtain a sworn disclosure statement from two staff prior to their first day of employment. Evidence: The sworn disclosure on record for Staff #1, with an employment date of 8/30/23, was dated 11/28/23. The sworn disclosure on record for Staff #2, with an employment date of 10/10/23, was dated 11/28/23.
Plan of Correction: Sworn statements were on file. Sworn statements will be signed prior first day of employment.
