Inspector Notes
An unannounced Code Compliance inspection was conducted today with the Principal from 11:15 am to 12:45 pm. Eighty three children were present with 11 staff providing supervision. The ages of children ranged from 3 to 5 years. The Licensing Inspector was able to observe group activities, circle time, outside play and transitions.
Eight staff and six children's files were reviewed today. The area of non compliance is outlined on the violation notice.
Violations

Based on a review of 8 staff files and an interview with the Principal, it was determined that repeat background checks were not obtained every five years.
Evidence:
1. Staff A's Fingerprint background check was dated 3/14/18, their Central Registry background check was dated 8/14/15 and their Sworn Disclosure Statement was dated 7/8/15. None of these background checks have been renewed within five years of the original date.
2. Staff B's Fingerprint background check was dated 3/5/18 and it has not been renewed within five years.
3. Staff C's Fingerprint background check was dated 4/17/18 and it has not been renewed within five years.
Evidence:
1. Staff A's Fingerprint background check was dated 3/14/18, their Central Registry background check was dated 8/14/15 and their Sworn Disclosure Statement was dated 7/8/15. None of these background checks have been renewed within five years of the original date.
2. Staff B's Fingerprint background check was dated 3/5/18 and it has not been renewed within five years.
3. Staff C's Fingerprint background check was dated 4/17/18 and it has not been renewed within five years.
Plan of Correction: We will obtain new background checks for staff A, B and C.
