Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documentation review.

A code compliance inspection was initiated on 09/20/2021 and concluded on 09/21/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 22 children present, ranging in ages from two to four, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of three children?s records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable code sections and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to have a completed central registry record check by the end of the 30th day of employment.

Evidence:

1. The record for staff 2, start date 11/16/20 did not contain a central registry background check.
2. The director verified they were unable to find a central registry background check for staff 2.
Plan of Correction: The documentation for central registry background check for staff 2 was completed and mailed on 9/20/21. In the future staff records will be organized in a manner to ensure all paperwork is maintained and collected as required.
