Inspector Notes

An unannounced code compliance inspection was initiated on 12/1/2022 and concluded on 12/1/2022 from 9:40 AM to 11:00 AM. There were 22 children present, ranging in ages from three to five, with five staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of two children?s records and three staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to ensure all staff have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him/her from caring for children under his/her supervision prior to start date.

Evidence:

1. The record for staff 1 was reviewed. The start date was documented as 4/20/22. The record did not contain a staff health report.
2. The administrator stated she did not have a staff health report for staff 1.
Plan of Correction: Staff 1 will be required to provide a staff health report as soon as a doctor appointment can be scheduled. In the future no staff will be allowed to start working until the documentation of a staff health report is received.

Based on a review of the staff handbook and interview, the center failed to implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards.

Evidence:

-In the 2's room, Clorox wipes & disinfectant spray were on the child height counter beside the sink and in the cabinet under the sink. The cabinet did not have a lock.
- In the 4's room, Clorox wipes and Clorox Clinical Germicidal Cleaner plus Bleach were beside the sink accessible to children.
- In the 3's children's bathroom on the windowsill, was a spray bottle of disinfectant spray.
- In the cafeteria, there were multiple cleaners such as bleach in unlocked bottom cabinets accessible to children.
- The director stated the center's procedures for hazardous substances are as follows:
All hazardous substances like cleaning supplies are placed in cabinets that are out of reach of the children or are locked in the rooms. In the event a room has all that it needs to sustain, the extra materials are marked according to the room and placed in a locked cabinet on the second floor of the building. The church employs a Janitor to clean the facility after hours. All of their materials are also locked in a room on the second floor.
Plan of Correction: All hazardous substances will be moved to a location that is inaccessible to children. Staff will be retrained on the center's procedures regarding hazardous substances.
