Inspector Notes

An unannounced monitoring inspection was conducted on 04/30/2025 as a part of the licensure period. The on-site inspection began at 12:00pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 85 children present and 10 staff. The inspector reviewed 3 children?s records and 3 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Staff are required to obtain repeat background checks within the past 5 years. Staff B's did not have the results of a central registry check within 5 years of the last one which was dated 11/25/2025.
Plan of Correction: Staff B's central registry check will be sent out this week. The results will be documented in the staff file.
Standard 8VAC20-780-160-C
Staff are required to obtain a tuberculosis (TB) screening at least every two years from the date of the first initial screening or testing.

Staff C did not have a repeat TB screening. The last documented TB screening was dated 10/19/2022.
Plan of Correction: Staff C will be sent to obtain a TB test/screening this week. The results will be put in the staff file.
Standard 8VAC20-780-540-E
The center is required to have nonmedical emergency supplies. The center did not have a working radio or batteries.
Plan of Correction: The center will get batteries and test the radio to ensure it works.
