Inspector Notes

An unannounced monitoring inspection was conducted on 09/23/2025 as a part of the licensure period.The on-site inspection began at 11:00am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. The inspector reviewed 5 children?s records and 3 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Staff are required to submit fingerprint results prior to being hired. Staff #2 (hired 09/2025) did not have documentation of having fingerprints.
Plan of Correction: The program director has tried to get her fingerprinted but has run into issues submitting and receiving results. The director has contacted the Office of Background Check Investigations to resolve the issue and obtain staff #2's fingerprints.

Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. The preschool classroom had an unlocked cabinet that had 2 disinfectant sprays and one multipurpose cleaner.
Plan of Correction: The program director will remind staff to put the safety locks on the cabinets after every use.
