Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was initiated on 05/05/2021 and concluded on 05/05/2021. The director was contacted by telephone to initiate the inspection. There were 33 children present and 10 staff. The Inspector reviewed 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable code sections and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, a sworn statement or affirmation was not completed before employment for 2 of 3 staff.

Evidence:
1. The Sworn Statement in the record for Staff #1 (date of employment: 8/24/2020) was dated 9/1/2020.
2. The Sworn Statement in the record for Staff #2 (date of employment: 2/13/2020) was dated 9/3/2020.
Plan of Correction: Sworn statements for both staff members have been obtained.

Based on record review and staff interview, results of a sex offender registry check and a search of the child abuse and neglect registry from any state in which the individual has resided the preceding five years were not obtained for 1 staff.

Evidence:
1. The Sworn Statement in the record for Staff #2 (date of employment 2/13/2020) indicated Staff #2 previously resided in Maryland.
2. Staff stated they last lived in Maryland in 2016.
3. The record for Staff #2 did not contain results of a sex offender registry check and a search of the child abuse and neglect registry from the state of Maryland.
Plan of Correction: Documentation that the requests were sent will be provided to the Department within the next 10 business days.
