Inspector Notes

An unannounced renewal inspection was conducted on February 26, 2024 between the hours of 9:15am and 11:25am. There were 2 classrooms observed with a total of 12 children present ranging in ages from 3 to 5 years with 5 staff supervising. Ratios were in compliance.
The children were observed playing with manipulatives, eating snack, and other organized activities. A complete inspection of the physical plant, child and staff records, fire drill log, and emergency procedures were observed during this inspection. No medication on sight at the time of inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, there was no follow up on an Out-of-State Child Abuse and Neglect Search 45 days after the request to search.
Evidence:
1. The record for Staff #2 did not have documentation of a follow-up on the Out-of-State Central Registry Search that was submitted on 7/22/23.
2. The director (Staff #5) confirmed there was not a follow-up completed on the request.
Plan of Correction: I will follow-up today on the Out-of-State Central Registry search. I will ensure this is completed for any new employees.
