Inspector Notes
An unannounced monitoring inspection was initiated on June 11, 2025 and completed on June 30, 2025 in response to a self-report received by the licensing office on June 11, 2025 relating to supervision, special care provisions, and programs. This inspection included document review and interview.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 7, 2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. During a field trip, several school-aged children were denied access to a bathroom upon request. Child #1 cried when denied access and told to hold it. Child #1 (age 10 years) and Child #2 (age 7 years) had toileting accidents, urinating on themselves. Staff #1 was heard saying, a child that is 10 years old shouldn?t be peeing themselves, there is nothing they could do, and the child should not have had access to the child?s water bottle during the bus trip. Both children had to remain in soiled clothing for the remainder of the field trip, which was approximately 8 hours, as they walked around the zoo, ate lunch and traveled on the bus.
Plan of Correction: An all-staff meeting was held on 06/23 where many things were discussed, including the staffs right to stop a bus on a trip for any reason. A bathroom accident policy has been developed that provides staff guidance on how to deal with these situations. It also stresses that these situations need to be kept confidential and the child should be reassured it was not their fault. Lastly, staff will be required to take three different trainings: 1. Ethics - A guide for Professional Behavior 2. Positive Teacher-Child Interaction Strategies 3. Talk and Think about Health Policies and practices.
Standard 8VAC20-780-400-A
In order to promote the child?s physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort, support, and respect personal privacy. Several children were denied access to a restroom upon request, resulting in two children (ages 7 years and 10 years) having toileting accidents. When informed that child #1 had urinated themselves, staff #2 replied that the child was ?going to be a bit uncomfortable then, sorry? and staff #1 replied, ?oh well." Staff discussed the incident in the presence of a bus full of children, with staff #1 loudly stating child #1 shouldn?t be peeing on themselves. Neither child was offered dry clothing and had to remain in soiled clothing for the duration of the field trip.

Standard 8VAC20-780-500-B
When a child?s clothing becomes wet or soiled, the child shall be cleaned and changed immediately. On June 10, 2025, child #1 (age 10 years) and child #2 (age 7 years) were forced to remain in wet, soiled clothing from approximately 9:30 a.m. to 5:43 p.m
Plan of Correction: Clothing of various sizes has been purchased for the sites to take on field trips. Additionally, a request will be going out to parents to request donations of clothes their children may have outgrown to increase our supply.
