Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/23/2025 and completed on 09/23/2025. The on-site inspection began at 10:05 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 4 staff. The inspector reviewed 5 staff records and 5 children's records on-site on 09/23/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

All employees shall undergo a background check every five years thereafter.
The most recent fingerprint background check for staff 1 was obtained on 09/08/2020 which is more than five years ago.
Plan of Correction: Staff member will complete fingerprinting for renewal.
Standard 8VAC20-780-350-O
The ratio for balanced-mixed-age groupings of children shall be one staff member for every 14 children provided the enrollment in the balance mixed-age grouping comprises a relatively even allocation of children in each of three ages of three to six years.
1. There were 11 children with 1 staff in Children's House 1 and Children's House 2. The ages and allocation of the children in each group did not meet the definition of a balanced mixed age grouping as there were no five or six year old children in either group.
a. The allocation of children in CH 1 included 4 three-year old children and 7 four-year old children;
b. The allocation of children in CH 2 included 2 three-year old children and 9 four-year old children.
Plan of Correction: When Kindergarteners are on a field trip, an extra staff member will be added to the classroom if classroom has more than 10 students remaining.
