Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 09/23/2025 and completed on 09/23/2025. The on-site inspection began at 10:05 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 4 staff. The inspector reviewed 5 staff records and 5 children's records on-site on 09/23/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Violations
2The most recent fingerprint background check for staff 1 was obtained on 09/08/2020 which is more than five years ago.
1. There were 11 children with 1 staff in Children's House 1 and Children's House 2. The ages and allocation of the children in each group did not meet the definition of a balanced mixed age grouping as there were no five or six year old children in either group.
a. The allocation of children in CH 1 included 4 three-year old children and 7 four-year old children;
b. The allocation of children in CH 2 included 2 three-year old children and 9 four-year old children.
