Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.

An unannounced renewal inspection was conducted at 11:18am. The program director was contacted by phone to initiate the inspection. There were 43 children and 10 direct-care staff present. Ratios were in compliance at the time of the inspection. Children were observed washing hands, playing on the playground, playing with dough, blocks, tanagrams, writing, and listening to an audio book. The facility appeared to be clean, and classrooms were sufficiently supplied with toys and equipment for the children. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on records reviewed, it was determined that 2 of 3 staff records did not include out-of-state background checks, as required.

Evidence:

1. Staff #1 - There was no child abuse and neglect search results and criminal record check for one State, and no sex offender registry search for 2 States that the individual has resided.
