Inspector Notes
Conducted an annual religious exempt renewal inspection at 10:45am. Observed 40 children + 8 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include: name writing, coloring, playing with puzzles and play dough, reading books and having story-time, and active outdoor play. The facility was found to be clean and well maintained inside and outside. Classrooms were well supplied with toys and equipment for the children. Staff were actively engaged with the children and activities in all groups. An area of non-compliance was found with standards and laws reviewed. Based on records reviewed, it was determined that 1 of 5 staff records did not include an out-of-state background check prior to the first day of employment. Evidence: Staff #1 was hired in 2019 and to-date does not have a completed out-of-state criminal record check.
Plan of Correction: A new sworn disclosure statement was completed to show out of state background was not needed as it is outside the 5 year window. A new background check will be performed in May 2023 on [name of staff #1]. When the new background check is completed there will no longer be any out of state background checks as she has been in the State of VA since May 2018 (5 years). A new sworn statement will be completed as well.
