Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on September 23, 2021 and concluded on September 30, 2021. The staff person in charge was contacted by email to initiate the inspection. There were nine children present and two staff. The inspector emailed the staff person in charge a list of items required to complete the inspection. The Inspector reviewed three children?s records and two staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, in one of two staff records reviewed, the licensee did not ensure that staff resubmit TB test results every two years.

Evidence: The staff person in charge verified that the most recent TB screening for Staff #2 was more than two years old and was dated 8/2/19.
Plan of Correction: The staff member has a TB test scheduled for next week.
