Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program. A monitoring inspection was initiated on August 16, 2022 and concluded on August 22, 2022. There were 32 children present, ranging in ages from 6 to11, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on review of 5 staff records and staff interviews, the licensee did not ensure each staff member shall submit documentation of a negative tuberculosis screening and the documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: The record for staff #4 date of hire June 24, 2022 did not documentation of TB screening/ test
Plan of Correction: A request will be made for a physical copy of the negative tuberculosis test docuemtniaon to be kept on site.
Standard 8VAC20-780-510-B
Based on medication review, documentation review and staff interviews, the licensee did not ensure that prescription medication shall be given to a child only with written authorization from the parent.

Evidence; The medication log for Child #4 was reviewed and medication (methylphenidate 5mg) had been administered a total of 12 times between June 15, 2022 and July 27, 2022. The medication authorization for Child #4 was reviewed and the parent had not authorized administration of the medication.
Plan of Correction: Medication forms will be more carefully reviewed to ensure the parent provides a signature along with the other information they must fill out.
