Inspector Notes

An unannounced, on-site renewal inspection was initiated and concluded on 5/29/2024. The on-site inspection began at 4:25 pm and ended at 5:10 pm. The inspector reviewed compliance in the areas listed above. There were 15 children present and 4 staff. The inspector reviewed 5 children?s records on-site and 4 staff records electronically on 5/30/2024. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/10/2024. A POC submitted after this date will not appear on the public website.

The center is required to obtain out of state background checks from any state in which a staff member has resided in the preceding five years. There were no out of state background checks completed on staff #3 for the states of Connecticut and Washington.
Plan of Correction: Staff member misunderstood paperwork. They lived out of state over eight plus years ago. They will complete and updated form.
