An unannounced, on-site monitoring inspection was initiated on 8/14/2024 and completed on 8/19/2024, The on-site inspection began at 9:50 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 37 children present and 4 staff. The inspector reviewed 5 children?s records on-site and 4 staff records electronically on 8/19/2024. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #2 updated screening was due 1 month ago. The updated screening has not yet been completed.
Plan of Correction: Staff will get a TB test as soon as possible.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #3 was missing 1 required component and the record for Child #5 was missing 2 required components.
