Inspector Notes

A monitoring inspection was initiated on February 15, 2023 and concluded on February 23, 2023. There were 25 children present, ranging in ages from 6 years to age 11, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of 3 staff records and staff interviews, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening and the documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: Staff #3?s date of hire was 1-9-23 and the TB screening/ test was dated 1-23-23 and it was not conducted within the required timeframe.
