Inspector Notes

An unannounced renewal inspection was conducted today from 10:30am-1:00pm. There were 42 children directly supervised by 9 staff. The physical plant, 4 staff records, 5 agent records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed engaging in free play, making Valentine's Day crafts, playing with blocks, identifying and writing letters, playing outside, and eating snack. Hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Based on record review, 1 of 5 children's physical examinations was not completed within the time period required.

Evidence:
1. Child #1 was 2 years 9 months old when they began attending the center on 09/07/2022.
2. Physicals for children ages 2 years through 5 years must be completed within 12 months prior to attendance.
3. The physical examination in the record for Child #1 was dated 07/05/2021.
Plan of Correction: Staff will review date requirements for physicals.

Based on record review, the center did obtain the proof of identity for one child within seven business days as required in ? 22.1-289.049 of the Code of Virginia.

Evidence:
1. Child #1 began attending the center on 9/7/2022.
2. Documentation in the record for Child #1 indicated the proof of identity was viewed on 9/26/2022.
Plan of Correction: Staff will receive a review and retraining of requirements for verifying proof of identity for children.
