Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 10:30am-1:30pm. 23 children were observed directly supervised by 5 staff. The physical plant, 3 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing freely and having lunch. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain a fingerprint-based criminal history record check every 5 years for one staff.

Evidence:
1. The fingerprint check in the record for Staff #1 was dated 08/17/2023. The previous fingerprint check in the record for Staff #1 was dated 6/11/2018, more than 5 years prior.
Plan of Correction: Repeat backgrounds will be completed within 5 years.
