Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review.
A monitoring inspection was conducted on January 13, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 13 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289-036-B-4
Based on record review, the child day center did not obtain a copy of the results of a a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: The record for Staff 1 did not contain the results of a central registry check or a sex offender registry check from a state the staff member has resided in within the preceding five years.
Plan of Correction: DOJ National Sex Offender website was checked 1/24/2022.
Child Abuse Registry of NJ online form was submitted 1/24/22 with employee's addresses back to 1980 as required by NJ.
Future out of state hires will be checked using info supplied by inspector prior to hire.
Standard 22.1-289-036-E
Based on record review, two staff members began working at the child day center prior to the results of a national criminal record check.
Evidence:
1. Staff #1 has a date of hire documented as 8/14/2021. Results of a national fingerprint criminal record check were received on 8/30/2021.
2. Staff #2 has a date of hire documented as 8/26/2021. Results of a national fingerprint criminal record check were received on 9/8/2021.
Plan of Correction: 8/14 and 8/26 were the offer dates when paperwork was given to prospective employees to be completed before start of employment 9/7.
No children were onsite until 9/13. In future, files will indicate start date after paper completion and results returned.
