Inspector Notes
An unannounced monitoring inspection was conducted on May, 5, 2022. The physical plant, 4 staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside, and eating lunch from home and a school supplied snack celebrating the Cinco de Mayo holiday. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

4Evidence:
Outlets without protective covers were observed in the hallway areas of the area of the recreation center that is used for licensed child day programs.
Evidence:
A staff member stated that she recently administered authorized medication to Child 4, but did not document the occurrence.
Evidence:
Two medications for Child 4 were present in the center on the date of inspection The authorization for each medication expired on 3/30/2022.
Evidence:
Child 1's record did not contain the addresses for two persons to contact in the event a parent cannot be reached.
