Inspector Notes

An unannounced monitoring inspection was conducted on May, 5, 2022. The physical plant, 4 staff records, 3 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside, and eating lunch from home and a school supplied snack celebrating the Cinco de Mayo holiday. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on observation, not all outlets had protective covers installed.
Evidence:
Outlets without protective covers were observed in the hallway areas of the area of the recreation center that is used for licensed child day programs.

Based on staff statement, the center did not keep a record of all medications given to a child to include the child's name, the amount and type of medication, the date and time of administration, the name of administering staff member, or any adverse reactions to the medication.
Evidence:
A staff member stated that she recently administered authorized medication to Child 4, but did not document the occurrence.

Based on a review of medication and documentation, medication was not returned to the parent within 14 days of the medication authorization expiration date.
