Inspector Notes
An unannounced renewal inspection was conducted today from 10:40am-12:00pm. A portion of the inspection was conducted with county officials via email to obtain background check information. The physical plant, 7 staff and applicant records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, and eating lunch brought from home. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Applicant 3's record contained results of a fingerprint criminal record check conducted on 7/26/2017.
Evidence:
1. Applicant 3's sworn disclosure was dated 10/19/2022.
2. Applicant 2 and 3's record did not contain the results of a central registry check.
4. Met with HR on new procedures for getting results.
Evidence:
Staff 2 and 3's record did not contain the results of a central registry background check with in 30 days of employment.
Evidence:
Child 3 did not have a written allergy plan despite having a documented diagnosed food allergy.
Evidence:
The findings from the center's 5/5/2022 inspection were not posted in the facility on the date of inspection.
