Inspector Notes

A complaint investigation was conducted on 07/14/2023. The inspector arrived at 11:00 AM and departed at 2:00 PM. The complaint alleged parents are not being notified of children's injuries and reports are not being completed on the day of the injury. The inspector reviewed compliance in the areas of administration, staffing and supervision and special care. The investigation included observation, interviews and review of documentation.

Information gathered during the investigation determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon review of records, the facility has not ensured that a written record of children's injuries is written the day of occurrence and includes all required information.
Evidence:
1. An injury record from 5/25/2023 for child 1 did not include the date, time and how the parent was notified of the child's injury.
2. A second injury record for child 1 on 5/25/2023 did not include how the parent was notified of the child's injury.
3. An injury record for child 2 dated 6/1/2023 did not include the date and time the parent was notified of the child's injury.
4. An injury record dated 3/1/2023 for child 3 did not include the time the parent was notified of the child's injury.
