Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.

A renewal inspection was initiated on 11/10/2021 and completed on 11/15/2021 with an in-person inspection. There were 52 children present, ranging in ages from 2 years - 5 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 children's records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to obtain the results of a fingerprint background check prior to employment.

Evidence:
1. The results of a fingerprint background check were not available for staff 1. Staff 1 began employment on 08/26/2021.
2. Administrative staff and Staff 1 stated that a fingerprint background check had been requested however, no results were available.
Plan of Correction: A criminal record check was requested and the results received with no conviction data on 08/16/2021.
Staff 1 did submit to fingerprinting on 08/05/2021 however, the results were never received.
OBI was contacted however, the results are no longer available in their system. Staff 1 has an appointment for fingerprinting on 11/17/2021.

Based on record review and interview, the center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. The results of a central registry check and sex offender check is not available from the State of Maryland - where Staff 1 has resided within the past five years.
a. Administrative staff verified that a central registry check had not been requested from the State of Maryland.
2. A sex offender check and criminal record check is not available from the State of South Carolina - where staff 2 has resided within the past 5 years.
a. Administrative staff verified that a sex offender check and criminal record check had not been requested from the State of South Carolina.
Plan of Correction: All of the out-of-state background check forms have been completed for staff 1 and staff 2. These requests were submitted on 11/16/2021.
