Inspector Notes

An unannounced monitoring inspection was conducted on 05/24/2022 from 9:40 am - 11:15 am. At the time of entrance there were 53 children in care with 12 staff. The children ranged in age from two years - five years. Children were observed during outdoor play, snack and morning program time. Records were reviewed for three children and three staff.
Violations were observed in the areas of staff background checks and staff TB screening. These violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Based on record review and interview, the center failed to obtain a copy of the results of a fingerprint background check prior to employment.

Evidence:
1. The results of the fingerprint background check for staff 1 were obtained after her hire date of 03/01/2022.
a. The fingerprint check was obtained on 03/10/2022.
Administrative staff confirmed that staff 1 began work prior to receipt of the fingerprint background check.
Plan of Correction: The director stated:
Staff 1 was not left alone with children and was always supervised by a staff person with a current background check.
In the future, all staff will have a completed fingerprint background check prior to beginning work.

Based on record review and interview, the center failed to ensure that each staff member shall submit documentation of a TB screening at the time of employment and prior to coming into contact with children.

Evidence:
1. Staff 1, hire date 03/01/2022, lacks documentation of a TB screening.
2. Administrative staff confirmed that staff 1 had not provided documentation of a current TB screening.
Plan of Correction: The director stated that staff 1 was asked to obtain a TB screening on the day of the inspection. Staff 1 will obtain a TB screening as soon as possible.
