Inspector Notes

An unannounced monitoring inspection was conducted today at 12:00pm. The physical plant, 5 staff records, and policies were inspected. Children were observed participating in group play, playing outside, watching a bubble show, and singing prayers. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review, daily simple health screening and exclusion of sick children was not conducted by a person trained to perform such screenings.
Evidence:
On the date of inspection no staff records contained documentation that they have been trained to perform daily health screenings.
Plan of Correction: A member of our team will be completing the appropriate training and re-up it every year. Stephanie ensuring training is completed.

Based on record review, the center did not obtain the results of a central registry background check for staff who have resided in a state other than Virginia in the preceding five years.
Evidence:
Staff 5's personal statement confirms residence in a state other than Virginia in the preceding five years. The staff member's record did not contain the results of a central registry for the listed state of residence.
Plan of Correction: Background check to be completed for previous MD resident. Future bgr. checks to be confirmed re: which states need to be checked.
