Inspector Notes

A Renewal inspection was initiated today. This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. The director was contacted by Zoom Meeting and a virtual inspection was conducted. There were a total of 4 children present with 7 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and staff 4 records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations and were documented on the violation notice issued to the program. Standard 22VAC40-185-330-B
Based on a virtual observation of the playground area, adequate resilient surfacing was not maintained.
Evidence:
The area below the slide and the climbing bear structure did not have a minimum of six inches of resilient surfacing. Several other areas of the playground to include fall zones did not have adequate resilient surfacing that comply with minimum safety standards.
Plan of Correction: We have requested a new delivery of mulch but do not know when the delivery is scheduled for. We will replenish the mulch prior to the holiday break.

Based on review of child records, documentation of viewing proof of the child's identity and age was not found for 2 out of 4 records reviewed.
Evidence:
Child #1 and #3's record had a pdf document uploaded in an electronic file but the center was unable to open the file on site to verify the identity and age of the children.
Plan of Correction: We will need to have access or maintain a hard copy of the review of this documentation
