Inspector Notes

An unannounced Renewal inspection was conducted today. A total of 17 children with 4 staff were onsite. Two classrooms were currently occupied during the inspection. The children were observed in the upstairs classroom for 4 year olds free playing on the floor and also playing with playdoh. The classroom was decorated with projects such as stained glass leaves and other seasonal crafts. The classroom downstairs were transitioning to circle time where they were seated quietly listing to the teacher read "My Book of Friends". Holiday tree handprints that required the children to paint their hands green and handprint the shape of holiday trees on canvas was drying on the counter top. Morning snack is brought from home. Lunch boxes were labeled and dated and hung in their cubby locations outside of the classroom. No identified food allergies at this time.

A total of 5 children's records, 4 staff records and current certifications for staff were reviewed. Annual training, staff qualifications and background checks are reviewed at the administrative office on an annual basis. This inspection occurred between 10am and 11:30am.

Based on review of staff records, a TB screening was not obtained within 30 days of hire.
Evidence:
Staff #1 was hired on 08/18/2023. The TB screening on file was dated 01/31/2023.
Plan of Correction: All new hires will have their TB screenings completed within 30 days of their hire date.

Based on review of documentation, an annual fire inspection report was not provided to the licensing representative.
Evidence:
The annual fire inspection report was not available.
Plan of Correction: We will obtain a copy of the fire inspection and will forward it to the Licensing Inspector for review.
Standard 8VAC20-780-80-B-2
Based on review of injury reports, the center failed to inform the Department's representative within 2 business days of an injury to a child that required outside medical attention.
Evidence:
1. On 06/26/2023, Child #4 sustained an injury which required outside medical attention.
2. The injury was not reported to the Department.
Plan of Correction: Any injury sustained at the school that requires outside medical attention will be reported to Licensing on the online portal.
